Facility Evaluation Report
On 12-2-2025 at 10:40am, Licensing Program Analysts (LPAs) Michael Bilger and Sommer Hayes arrived unannounced to conduct a case management visit regarding an incident reported by facility on 10-26-2025. LPA met with Administrator Christopher Walters and explained the purpose of the visit. LPA reviewed incident report with Administrator and conducted a brief interview with Administrator. LPA also reviewed individual service plan (ISP) for resident1 (R1) and police call log dated 10-26-2025. Based on record reviews and interviews, on 10-25-2025 at approximately 10:00pm, a police officer arrived at facility to follow up on comments made by R1 on 10-24-2025 stating he was "touched inappropriately" by his "brother". After police officer spoke with R1 it was revealed that R1 stated R2 touched him inappropriately 2 weeks prior. On 10-26-2025, police officer arrived at facility to continue investigation and it was concluded that both R1 and R2 denied the occurrence. Licensee also reported incident to the Department and Ombudsman within regulatory time frames.
Police call log reviewed states R1 has a history of desiring police involvement and making falsified accusations against other residents. LPA reviewed ISP which indicates R1's history of falsified statements with interventions in place. As a result of today's case management, no citations are issued. An exit interview was conducted with Administrator and a copy of this report was provided.
