Facility Evaluation Report

On 5-14-2026 at 2:00pm , Licensing Program Analysts (LPAs) Michael Bilger and Albert Johnson arrived unannounced to conduct a case management visit regarding new clients placed at facility on 5-13-2026. LPAs met with Administrator Diamond Curtis and explained the purpose of the visit. During todays visit LPAs conduct visits with resident1 (R1), R2, R3, and R4, R5, and R6. Based on visits conducted, it was determined that all residents noted above moved into facility on 5-13-2026 and have in possession all necessary personal belongings and medications in place. LPAs observed residents to be participating in regular facility functions including activities and mealtime procedures.

As a result of today’s visit, no citations are issued. An exit interview was conducted with Administrator and a copy of this report was provided.
