Complaint Investigation Report

The facility has a resident council meeting monthly. The facility has an activity agenda, which residents sign at the time of an activity. LPA Martinez observed all clients signatures on activity agendas. Also, the facility is visited by a recreation therapy consultant monthly. LPA Martinez reviewed client 1 (C1) monthly nursing summaries. Monthly summaries are from March 2024 to June 2024. July's monthly summary will be conducted on August 1, 2024. The summaries are maintained with the current information. Staff 1 reported C1 had no recent health changes that would require their assessment to be updated.

Due to the above noted information, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, and therefore the allegations are unsubstantiated.

An exit interview was conducted, and a copy of this report was provided to the facility.
