Facility Evaluation Report

On 4/24/2024, Licensing Program Analyst (LPA) Albert Johnson arrived to the facility and met with Shawn Flanagan and Farina Khan to follow -up on requested information from 4/22/2024. Later Joined by Leah Zubiate .

During the visit on 4/22/2024, the department requested information for staff including personnel files and other information for Staff S1, S2 and S3's Personnel Records (LIC501), training's, Mandated Reporter Forms, any staff write-ups: positive or negative, and employment applications. The facility submitted information for one staff (S1), however the above requested information was not sent by close of business on 4/23/2024 for the other two(S2 and S3). Today, the facility will provide the requested information at the close of this visit.

The facility did not have the records available for review as required by 80066(e)(1) personnel records and 80044(A)(b1)(c1) inspection authority.

As a result of this visit, deficiencies will be cited on 809-D, per Title 22 Regulations, Division 6.
