The inspector’s account

On 9/6/23 at approximately 2:20pm Licensing Program Analyst (LPA) Maja Jensen arrived at the facility unannounced to conduct a case management related to an incident that was reported by telephone. LPA Maja Jensen met with site supervisor Craig Vincelet and explained the purpose of today's visit.

On 8/28/23 at approximately 4:52pm Craig Vincelet called LPA Jensen and made a verbal report regarding the unauthorized absence of Resident 1 (R1). On 9/6/23, prior to commencing this visit LPA Jensen reviewed all incident reports received for August 2023 and September 2023 and confirmed that no written report was received by the Department for this incident. LPA Jensen interviewed site manager Craig Vincelet who also confirmed that a written report was not sent prior to this visit.

LPA Jensen interviewed site manager who stated R1 signed out indicating he is going to teh store, R1 did not return after 24 hours and then facility protocol was followed wherein a missing persons report was filed with law enforcement. After filing the missing persons report, site manager located R1 walking on a city street and brought R1 back to the facility on 8/31/23. LPA Jensen reviewed the sign out log and confirmed that R1 signed out on 8/25/23 at 3:20pm. The site manager confirmed that an incident report will be submitted to the Department for review on this date. The Department, after reviewing the report, may return to address any deficiencies found.

Deficiencies are being cited from the California Code of Regulations (CCR). Failure to correct may result in the assessment of civil penalties.

An exit interview was conducted and a copy of this report was given.
