08/18/2022 and conducted by Evaluator Maja Jensen
COMPLAINT CONTROL NUMBER: 27-AS-20220818161838
On 9/29/22 Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannouced to deliver findings for a complaint investigation related to the above listed allegation. LPA Jensen met with Craig Vincelet and explained the purpose of today's visit.
During the course of the investigation LPA Jensen reviewed training records for 3 staff members that served in 3 different roles. All staff for which records were reviewed were determined to have adequate training. The allegation is found to be UNSUBSTANTIATED. A finding of unsubstantiated means that while the allegation may be true, the preponderance of evidence standard has not been met. No deficiencies are being cited.
An exit interview was conducted and a copy of this report along with appeal rights were given.
