Complaint Investigation Report

LPA Jensen interviewed 7 staff members and 3 residents. The staff members interviewed consisted of a combination of former and current staff. The majority of those interviewed denied food was withheld from residents. On 9/19/22 during the course of the visit, LPA Jensen observed 3 residents request snacks outside of the regularly scheduled snack times and 3 of 3 residents were given snacks without hesitation. Based on interviews conducted and observations made during the course of the visit, the allegation that staff withhold food from residents is UNSUBSTANTIATED.

Meals are not planned with consideration for cultural and religious background and food habits of residents

The majority of parties interviewed denied having a problem with meals not being planned with consideration for cultural or religious preference. Based on the interviews the allegation is UNSUBSTANTIATED.

During the course of the visit on 9/19/22 LPA Jensen observed 3 random residents request snacks outside of regularly scheduled snacks times and snacks were given by staff without hesitation therefore the allegation is UNSUBSTANTIATED.

LPA Jensen interviewed 7 staff members and 3 residents. The staff members interviewed consisted of a combination of former and current staff. On 8/22/22 LPA Jensen tested the alert system with site supervisor in a resident's room. Staff responded to the room in a reasonable amount of time. On 9/19/22 LPA Jensen requested the alert system be activated in a randomly chosen room number while the LPA stood in the medication room with site supervisor Craig Vincelet.
