Facility Evaluation Report

On 7-16-2025 at 12:10pm, Licensing Program Analyst (LPA) Michael Bilger conducted a case management visit regarding an incident reported on 12-24-2024. LPA met with Administrator Michael Cooks and explained the purpose of the interview. LPA conducted brief interview with Administrator and reviewed incident report with administrator in addition to physician's report for resident1 (R1). Based on record review and interview, it was revealed that on 12-24-2024 at approximately 7:30am, resdient1 (R1) approached staff and informed them she took 24 pain pills while at a local store on 12-23-2024 at approximately 3:00pm. Emergency personnel were called as a result, and R1 refused to go to the hospital for evaluation. Based on review of R1's physician's report and interview, R1 was unable to leave facility unassisted. Based on interview, R1 informed facility staff of her intention to go to local store on 12-23-2024, and staff was not present with R1 resulting in an absence of supervision.

As a result of today's case management, citation is issued under Title 22, Division 6 and noted on LIC 809D. An immediate civil penalty of $500 is issued in addition to citation due to absence of supervision as noted above. An exit interview was conducted with Administrator and a copy of this report was provided. Appeal rights provided.
