The inspector’s account

Licensing Program Analysts (LPA) Kesha Lewis conduced a case management incident inspection regarding incident reports received dated 9/01/2025. LPA explained purpose to the administrator.

The incident reports were for R1 having suicidal ideology. LPA reviewed incidents reports Resident records including the physicians report and needs and service plan. All reporting was done on time and to the required departments. R1 was cleared by medical professionals and service plane was updated accordingly.

Per California Code of Regulations, Title 22 NO deficiencies were observed and are being cited during today's case management inspection.

An exit interview was conducted and a copy of this report was given via email.
