The inspector’s account

Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management incident inspection regarding incident reports received dated 5/16/2025. LPA explained purpose of visit to the administrator.

The incident reports were un-witnessed falls for R1 and bed bugs in the room of R2. LPA reviewed incidents reports Resident records. All reporting was done on time and to the required departments. Treatment were completed in a timely manner for R2 and re-treatment has been conducted.

Per California Code of Regulations, Title 22 NO deficiencies were observed and are being cited during today's case management inspection.

An exit interview was conducted and a copy of this report was given.
