Facility Evaluation Report

On 7-29-2025 at 1:38pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a case management visit regarding an incident reported on 4-21-2025. LPA met with Administrator Chris Chesney and explained the purpose of the visit. LPA reviewed incident report with Administrator during today's visit. Based on review of incident report, on 4-19-2025, resident1 (R1) engaged in using foul language towards a staff1 (S1), as well as an altercation with R2. Incident report further states R1 engaged in anxious behavior, in addition to hearing voices.

After the incident, facility staff arranged for R1 to be seen by primary physician. R1 is now current receiving psychological services monthly. Facility staff continues to maintain progress notes and address R1's behaviors including interventions.

Incident was reported by facility appropriately and per regulatory requirements. As a result of today's case management, no citations issued. An exit interview was conducted with Administrator and a copy of this report was provided.
