Facility Evaluation Report

On 8-4-2026 at 1:00pm, Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct a case management visit regarding the facilities' reported incident on 6/24/2026. LPA met with Program Director (PD) Yolanda Zequiera and explained the purpose of the visit.

LPA reviewed the incident report with PD as well as additional facility file documentation including: Report of suspected dependent adult /elder abuse (SOC341), general event report, in-service training(6/30/26) and correspondence between stakeholders. The following information was obtained through interviews and review of statements provided by program and transportation representatives.

R&D Transportation representative Cecelia Morales contacted Program Manager Danielle Hudson to report an incident that allegedly occurred on 06/24/2026 in the CVTC Stockton East parking lot. According to the information provided, while R1 was stepping off an MV Transportation bus, R1 dropped to their knees upon exiting the bus and onto the ground in the parking lot.
