Facility Evaluation Report
Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct an Annual. LPA met with Jerry Poole and explained the purpose of the visit.
LPA inspected the physical plant with including but not limited to the Conference Room, a large room known as General Assembly and other classrooms including outside areas LPA observed the facility to be free of odor, clean and in good repair. LPA observed sufficient furniture and lighting throughout the facility. There are no bodies of water present in/or around the facility. Hot water temperature was measured at 105 degrees Fahrenheit in client bathroom sink, which is within the required range of 105 to 120 degrees.
Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA observed centrally stored toxins are kept locked. A fire drill was conducted on 3/3/2026. Day program does administer medications to clients and these records were reviewed. LPA reviewed 10 client and 10 staff files, including criminal record clearances. All staff are fingerprint cleared and associated to the facility. First aid kit was checked and is complete.
No deficiencies were cited per Title 22 Regulations and the Health and Safety Code. Advisories given for restricted care plans, personal and incidental funds and food service.
