Facility Evaluation Report
on 12/9/25 on 10:25am, LPA Noel Wolf Petersen arrived unannounced, Cabrera Guest Home 2, is a adult residential facility with a capacity of 6 and a census of 3, serving adults aged 18-59 Level 3. No restriced conditions at this time. LPA met with administrator Eldred Cabrera.
Physical plant was inspected, including but not limited to the kitchen, bedrooms, bathrooms, common areas, storage areas, evacuation routes, and exteriors. Facility is clean, traffic areas are well lit, and unobstructed. Kitchen has lockable storage for sharps and toxics and medications, LPA would like the PRN/refusal/disposal of medication to have more complete documentation, including reason and effect. LPA Provided guidance that the back of the MAR record has an area to do this documentation. LPA Observed 2 days of perishable food, 7 days of nonperishable food. Bedrooms have adequate furniture and furnishings, LPA gave guidance that matress encasements would help with potential pest control measures. Bathrooms have functional hardware, LPA observed potentially toxic handsoap and gave guidance that pending the needs and services plans of the clients, that may be a hazard and its probably safer to get nontoxic soap. Evacuation gates are dragging, LPA gave guidance they should swing freely and latch closed and remain unlocked.
Fire extinguisher is dated 4/5/25, smoke/co alarm is functional, water temp measured at the sink is 113*F.
2 of 3 client records were reviewed, ipps, signed admission agreements, and recent 602s are present and up to date. Needs and Services plans are sparsley detailed, some fields are blank and filled feilds are general, LPA gave guidance to make detailed plans for addressing the needs of the clients. 4 of 4 staff files were reviewed for first aid/cpr, continued and inital training, and fingerprint/background clearance; documents are present and up to date. Administrator files including control of the property, surety bond, workers comp, liability insurance, evacuation control plan, facility posters, and infection control plan were present and up to date.
