Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct an case management visit on today's date. The LPA met with Staff Ruth Intong

The purpose of this visit was to verify if the appropriate people were notified about the sell of the business.

They have notified residents and or responsible parties and the regional center is aware. The facility is an adult residential facility (level 2) funded by Valley Mountain Regional Center with a current census of 6. Facility has 4 bedrooms and 3 bathrooms. Three bedrooms are for resident use. Facility has a formal dining area and a formal living room.

All required forms and notifications have been completed.

As a result of this inspection, no deficiencies were cited.
