Facility Evaluation Report

An Office Meeting was conducted on this day in the Sacramento South Regional Office via Microsoft Teams. The purpose of this meeting was to follow up with the facilities control of property and the forfeiture of license. Present in the meeting was Regional Manager Stephenie Doub, Licensing Program Manager (LPM) Lisa Rios, Licensee/Administrator Astrofel Escorido, Members of LIMM LLC: Ingrid Pil, Lorna Villanueva and Margarita King and Valley Mountain Representatives, Katina Richison, Erick Hernandez.

The Department learned that the licensee has requested a change of ownership (CHOW) application.

The focus of the concerns at this time include:

Control of Property - Licensees will provide a copy of the lease which Licensee reported is current and active

Administrator Designee - LIC200 and LIC308 will be received by the Department appointing someone.

VMRC and licensee confirmed Tropic Inc was vendored through VMRC and the new Licensee /vendor will be transitioning to LIMM Care LLC

Licensee confirmed that the business was sold.
