Facility Evaluation Report
On 3/3/2026, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a quarterly visit. LPA met with Licensee Esther Gapasin and explained the purpose of the visit.
LPA was not able to review any records because all residents were removed from the facility by regional center on January 13th 2025. All staff was laid off on January 13th 2025 and have not been present in the facility since.
LPA conducted a tour of the physical plant and observed the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility. There were no residents or resident belongings observed in the facility.
An exit interview was held with Licensee and a copy of the report was provided.
