Facility Evaluation Report
On 04/21/25, Licensing Program Analyst (LPA) Kesha Lewis arrived unannounced to conduct a quarterly visit. LPA met with Licensee and explained the purpose of the visit.
LPA was not able to review any records because all residents were removed from the facility by Valley Mountain Regional by January 13th 20205. All staff was laid off on January 13th 2025 and have not been working in the facility since. One Staff member is staying in the facility and watching the home while it is empty.
LPA conducted a tour of the physical plant and observed the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility. There were nor residents or resident belongings observed in the facility.
An exit interview was held with Esther Gapasin and a copy of the report was provided.
