Upon further review and interview, it was revealed that there is no existing agreement between Licensee and R1 or R1’s responsible person to utilize this storage space for facility storage purposes.

As a result, the preponderance of evidence standard is met, and this allegation is SUBSTANTIATED. Citation is issued and noted on LIC 9099D. Administrator gave permission via phone for staff to sign and acknowledge in his absence. An exit interview was conducted with S1, and a copy of this report was provided to S1. LIC 811 provided. Appeal rights provided.
