Facility Evaluation Report

On this day a meeting was held via Microsoft teems to review the facility's compliance with the quarterly visits. in the meeting for the department was Licensing program analst (LPA) Kesha Lewis, Licensing program manager (LPM) Liza King, Regional manager Stephene Doub, and LPA Michael Bilger. Representing the facility was LIMBRICK, ANTHONY licensee.

At this time the department finds the facility to have been in compliance with the requirement's for the quarterly visits and the department has no concerns regarding the facility at this time. Therefore the quarterly visits will cease.
