Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to complete a Case management visit.

Currently there are no clients at the facility. The licensee has decided to surrender the license. The reason is lack of success in getting the Regional Center (VMRC) to approve the program plan.

The plan has been with the VMRC for two years and the licensee has not been given a timeline for the process to be completed after two years and the cost to operate on a license without any residents is not financially sound.

The Licensee will be submitting a letter to inform the department of the licensee's intentions and will be surrendering the license back to the department within the next two weeks.
