Facility Evaluation Report

Licensing Program Analysts (LPA) Kesha Lewis arrived at this facility unannounced to issue citations for failure to correct. LPA was met by staff and LPA spoke with licensee on the phone. LPA explained the purpose of the visit to Administrator and staff.

LPA and staff inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility to ensure compliance with Title 22 regulations.

Permission was given for the staff member that is present at the facility to sing the reports.

Citations are being issued for failure to correct the plans of correction from the 10/31/2024 visit.

Exit interview held with staff and copies of reports given.
