On 5-8-2026 at 1:30pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a case management visit. LPA met with staff member Perla Nival and explained the purpose of the visit. Administrator Arvin Davis was made aware of LPA's visit and purpose via phone. The purpose of this visit was to amend a report previously written on 4-15-2026 to reflect as a pre-licensing case management for the proposed license at this same location.
An exit interview was conducted with staff member and a copy of this report was provided.
