Facility Evaluation Report
On 3-12-2026 at 10:15am, licensing program analyst (LPA) Michael Bilger arrived unannounced to conduct a quarterly visit. LPA met with Administrator Arvin Davis and explained the purpose of the visit. Administrator was not present initially and arrived at 12:00pm. LPA conducted a facility tour and reviewed five client files. LPA also reviewed facility's LIC 500 and medication records for five clients in care.
LPA observed facility to be clean and sanitary with no foul odors. Facility is maintaining adequate amounts of food for clients in care. Facility's LIC 308 is complete and current. Facility's LIC 500 revealed administrator scheduled for at least 40 hours per week. A review of resident records revealed all required contents in place. Medication records revealed all signatures in place to indicate medication given on time and per physician's order.
Facility administrator is currently in process with his accountant to solve a suspended Franchise Tax Board (FTB) LLC status, and a payment plan is in process for outstanding civil penalty fees. At this time, Licensee is seeking a new license for this property under a different LLC.
No citations issued as a result of today's visit. An exit interview was conducted with Administrator and a copy of this report was provided.
