Facility Evaluation Report

Licensing Program Analysts (LPA'S) Kesha Lewis and Melina Oropeza arrived at the facility to conduct a case management and interview staff regarding medication training.This is a follow up visit from visit on 03/10/2026. LPA met with caregiver and was informed the Administrator had just left to buy wipes for the facility and would be right back.The purpose of the visit was explained.

Current census is 5 residents, 0 are receiving hospice, 4 of 5 are receiving home health care specifically PT, 4 of 5 are insulin dependent with a sliding scale. No current catheters, No current colostomy and No current Wounds.

LPA learned there was a new staff member that started 02/13/2026. and one staff member had left the facility.

LPA verified staff present are fingerprint cleared. LPA toured the facility which included a staff room with 2 sets on bunk beds, and 4 resident rooms. The exterior of the building shows one screen which needs to be replaced and a manicured lawn. The interior living area is clean. bathroom 2 has pink mold growing in the shower. The bathroom has the necessary DME. Facility has required linen, lighting and furniture.

Facility appeared clean and well kept otherwise.

LPA interviewed two (2) staff members that were on duty during the visit S1-S2.
