Facility Evaluation Report

Licensing Program Analyst (LPA) Kesha Lewis conducted a case management visit in order to follow up on documentation that was to be provided to the department regarding R1-R2. that was due to the department by 03/02/2025.

Documentation was not provided as of today's date. Therefore citations are being issued during todays visit

LPA conducted a tour of the physical plant and observed the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility to ensure compliance with Title 22 regulations.

Exit interview was conducted and a copy of the report and appeal rights was given.
