Facility Evaluation Report

Licensing Program Analyst (LPA) Kesha Lewis conducted a case management visit in order to follow up on documentation that pertains to complaint 27-AS-20250414094952 that was to be provided to the department regarding R1 that was requested by the department on 04/07/2025.

Documentation had not been provided as of the LPA'S visit to the facility. Additionally staff was unaware of who took R1 out of the facility or the general whereabouts of R1. Therefore citations are being issued during todays visit see 809D page.

LPA conducted a tour of the physical plant and observed the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility to ensure compliance with Title 22 regulations.

Exit interview was conducted and a copy of the report and appeal rights was given.
