Facility Evaluation Report
Licensing Program Analyst (LPA) Kesha Lewis LPA) Kesha Lewis arrived at the facility unannounced to conducted a case management regarding a visit from 04/16/2025. LPA was met by administrator Arvin Davis. LPA explained purpose of visit.
LPA learned that a resident R1 has a colostomy and staff is caring for it. During the visit conducted a tour of the physical plant and observed the kitchen, dining room, resident bedrooms; resident bathrooms, laundry area, living area and other common areas, as well as outside of the facility to ensure compliance with Title 22 regulations.

Exit interview was conducted and a copy of the report and appeal rights was given.
