Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management regarding the plan of correction for a visit at the facility on 04/16/2025. LPA explained purpose of visit to staff.
The facility failed to complete the plan of correction for citations issued during the visit on 04/16/2025.
Per California Code of Regulations, Title 22 deficiencies are being cited during today's case management inspection. See LIC 421.
