Facility Evaluation Report

On 4/29/2025 at 2:00pm Licensing Program Analyst (LPA) Michael Bilger conducted a facility visit today in an effort to conduct the annual inspection. Licensee Guardian House, LLC, is requesting to initiate a temporary hold on the license. The Licensee confirmed that no residents have been accepted in to care. An

inspection of the property confirmed that there are no current residents in care. At this time the

home is vacant. Community Care Licensing (CCL) has confirmed with the property owner that there has not been a loss of property and the current lease agreement is active with the licensee.

The facility is pending vendorization with Regional Center and has submitted an application to CCL for a

CHANGE of LOCATION. The Licensee agreed to the following:

-annual fees will be kept current (your PIN was provided to you today)

-no residents will be accepted unless the Department has been notified prior to admission

-a mandatory reinspection must take place prior to accepting any residents

-Licensee will remain available to the Department for contact by phone or email
