Facility Evaluation Report
Licensing Program Analyst (LPA) Noel Wolf Petersen arrived unanounced to conduct an annual inspection, the LPA met with administrator Cora Poier and explained the purpose of the visit. Alchor Guest home is a level three home for adults aged 18-59. one guest has a restricted condition of diabetes.
A Physical plant inspection was completed including but not limited to the cliend bedrooms, bathrooms, kitchen, common areas, storage areas, exteriors and evacuation routes. The facility is clean, traffic areas are well lit and unobstucted. The fire extinguisher is dated 2/18/25. Posters were of the adequate size and dimension. The bedroms have required funiture and furnishings, the bathrooms have all required hardware in good repair and a water temperature measured at 108*F. The exteriors and common areas have room for activies, the fire exit is in good repair. kitchen and storage areas have adequate storage for toxics, sharps, and medications.

Client files were reviewed for 602's, ipp's, Signed admission agreements, personal rights, and house rules. Staff files were reviewed background checks, fingerprints, and trainings, and First aid/CPr. All files are up to date and present,
two P+I cash holdings were checked at random, 2 clients mars and medications were checked at random, all found to be in agreement in documentation and holdings.
Administrator documents were reviewed including but not limited to the sureity bond, proof of ownership, facility license, administrator certificate, administrator training, liability insurance, fire clearance, and facility sketch.
No citations at this time, an exit interview was held, a copy of the report was read and given to staff.
