Facility Evaluation Report

LPA Albert Johnson arrived at the care home today and met with LPA met with Sandra Soth Program Manager, to conduct a plan of correction visit in relation to a citation issued during the annual inspection visit on 7/23/2026.

Citation issued: 82020 Fire Clearance All day programs shall secure through the licensing agency and maintain a fire clearance approved by the city or county fire department, the district providing fire protection services, or the State Fire Marshal.

The plan required the Licensee/ Administrator shall address the deficiencies cited and service fire extinguisher, fire risers etc.. and submit new tags by POC date showing corrections have been made. The program has made an appointment to have the fire risers serviced on 8/4/2026, the fire extinguisher has been serviced and the other action items have been addressed.

The citation has not been cleared for the fire risers. The program will submit photo evidence that the system has been serviced and is operational. The department may return to confirm the correction and provide a Letter of Deficiency Citations Cleared at a later date. Exit Interview conducted.
