Facility Evaluation Report
On 5/22/25 at 10:10 Licensing Program Analyst (LPAs) Noel Wolf Petersen and Micheal Bilger arrived unannounced to conduct the annual inspection visit. They met with administrator/licensee Abideene Mustapha, and explained the purpose of the visit. There are no clients in the current census and the first client won't arrive for some months.
Client Record review and client interviews were not conducted as part of this visit. Staff record review was not conducted a part of this visit.
Physical plant inspection was conducted, including but not limited to the kitchen, common areas, resident bedrooms, resident bathrooms, and evacuation routes, and exteriors. The facility is clean and traffic areas are well lit and unobstructed. The required furniture and furnishings were present and in good repair. Required posters were of the correct dimension and height. Nightlights, Smoke alarms and CO detectors are functional.
First aid kit had required items, fire extinguisher was dated 3/7/25, water temperature was measured within 105-120*F and air temperature was comfortable. The sharps and toxics and were locked away, medication storage existed in a lockable element. 2 days of perishable food and 7 days of nonperishable food existed.
Administrator record review had all necessary documents. Evacuation plan, infection control plan, surety bond and liability insurance were up to date. Administrators were interviewed on 812's to follow.
Per title 22 regulation no citations were issued as a result of this visit.
An exit interview was conducted and a copy of the report read and left with the administrator.
