Facility Evaluation Report
On 9/15/25, at 9:15, Licensing Program Analyst (LPA) Noel Wolf Petersen, arrived unannounced to conduct an annual inspection, A Growing Seed Residential. The LPA met with administrator, Topeka Jackson to explain the purpose of the visit. Facility is a 3 bedroom facility for 4 clients ambulatory maximum, serving adults 18-59, all female. No expected restricted conditions at this time. The current census is 0, waiting on vendorization, for 2ish years.
The Physical Inspection, including but not limited to the kitchen, common areas, bathrooms, bedrooms, exteriors, and evacuation routes. Kitchen food supply is low for meals for 4 people(2x3x4+7x3x4), two cans out of expiration, knives and razorblades stored unlocked, non-personal hygene toxics stored unlocked, 4 mouse traps indicating the presence of rodents. administrator addressed problems as they were brought up. bathroom hardware is funtional, water temperature was delivered in range. exterior has some spiders, damaged screens, facility will mop up the spiders, fix or replace screens. bedroom has adequate furniture and furnishings for mattress encasements. storage area in the garage has non client personal property should be cleared out. the fire extinguisher is dated 9/15/25.

Administrator records including but not limited to, Lic 500, evacuation control plan, Infection control plan, house rules, facility rules, surety bond, control of the property(Mortgage statement), the bonding agency for the liability insurance, sample admission agreement(vmrc), and program design( sample menu, sample activites, refund policy).
Technical violations issued, exit interview conducted, a copy of the report was read and given to staff. Administrator will let the LPA know when they are ready for accepting a client for a case management.
