The inspector’s account

Unannounced Plan of Correction visit made out to this facility on 03/06/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Jose Ventura, who was briefly interviewed at this time.

The purpose of this visit was to follow up on the deficiencies that were cited from a prior annual visit conducted on 02/05/2025. This visit was to follow up on the Plans of Correction that were due.

The following deficiencies were observed and cited on 02/05/2025:

Staff responsible for providing direct care and supervision shall receive and maintain current training in first aid and cardiopulmonary resuscitation from persons qualified by agencies including, but not limited to, the American Red Cross.

The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided when such observation reveals unmet needs. When changes such as unusual weight gains or losses or deterioration of mental ability or a physical health condition are observed, the licensee shall ensure that such changes are documented and brought to the attention of the resident's physician and the resident's responsible person, if any.

This facility did complete the Plans of Correction and provided all of the required forms and documents at this time.

Plan of Correction clearance letters were printed and copies were provided to the facility designated Administrator at this time.

There were no further deficiencies observed or cited during today's Plan of Correction visit. Exit Interview
