Facility Evaluation Report

Unannounced case management visit made out to this facility on 09/10/2025 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated Administrator, Monica Stelmack, who was briefly interviewed at this time.

There were also no staff on site at this time as well.

It was learned that the last date of service to the clients, and the presence of day program staff, were back on 06/30/2025. Since that time there have not been any new participants present nor any new staff employed.

This day program was also vendorized through Valley Mountain Regional Center (VMRC) at this time.

The purpose of this visit was to discuss with the facility designated Administrator about recent communications that were exchanged in regards to the future of this day program and the direction that it was heading towards.

A discussion was held with the facility designated Administrator at this time.

It was learned that she will be working with VMRC on changing the client base that was being served here at this time. It was learned that a date of 09/15/2025 was set with VMRC for this facility designated Administrator to conduct a presentation with them at that time.

It was learned that further evaluations will need to be made in order to decide whether this day program will remain as its own entity or join another one and, in doing so, will change the Licensee altogether.

A discussion was held with the facility designated Administrator as to what that would entail and what it would mean for this license.

This LPA will wait for the final decision from this facility designated Administrator before taking further action.
