Facility Evaluation Report
Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced 12:30 pm to conduct an annual inspection of Weldwoods, and Adult Residential Facility with a capacity of 6 and a current census of 3. No restricted conditions at this time. LPA met with administrator Bokime Etim and explained the purpose of the visit.
Physical plant inspection included but was not limited to the kitchen, bedrooms, bathrooms, common areas, exteriors, and evacuation routes. Facility is clean, traffic areas are well lit and unobstructed. the kitchen, has lockable storage for sharps and toxics and medications. two clients medications and mars were checked at random, they had accurate documentation for quantity and kind in the MAR. Bedrooms have required furniture and furnishings, Bathrooms have hardware that's functional, water is delivered between 105-120*F Evacuation route latches closed and swings freely, exteriors are clean. Neighboring additional unit is undergoing construction. It is seperated by chain link from the mainbuilding on the lot, a permanent fance will go up when construction is complete. The neighboring property is not going to be used to increase the capacity at this time, and the fire marshal is going to be coming by to update the fire clearance.
Fire extinguisher is dated May, 27, 2025. first aid kit has all necessary items, the smoke alarm is functional.
