Facility Evaluation Report

Unannounced case management visit made out to this facility on 09/10/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Bokime Etim, at this time. A brief interview was conducted with the facility designated Administrator at this time.

The purpose of this case management visit was to conduct a quarterly visit as outlined from the Informal Conference held on 11/22/2024 and the requirement to have increased monitoring at that time.

The focus of this quarterly visit was to review the following items that were initially brought forth on 11/22/2024:

The Administrator will be present at least 40 hours a week during the increased monitoring time period. These hours will be reflected on the updated LIC 500 provided to CCL.

Facility residents will be able to participate and hold resident council meetings on a regular basis. Proof of notes and minutes will be taken and provided to CCL.

Proof of facility resident outings, on a weekly basis, will be provided to CCL for review on a monthly basis.

A brief tour of the facility was conducted.

There were no deficiencies observed or cited during today's quarterly visit.
