Facility Evaluation Report
Licensing Program Manger (LPM) Liza King and Licensing Program Analyst (LPA) Noel WolfPetersen arrived to conduct the annual inspection. Upon arrival a phone call was made to notify the licenssee of the inspection. Approx 15minutes later the licensees husband arrived and provided access to the home.
The home is licensed for 4 ambulatory IDD clients vendorized through Regional Center.
The home although has not accepted its first client to date it is a new home, clean and well kept. All required furnishings are in place and the facility presents ready to accept its first client.
The licensee was informed that they should notify their LPA Charlie Yang 916-709-6507 upon notification of their first client being accepted and a reinspection will occur to complete the full inspection and provide any guidance.
Fire Extinguishers were expired and need to be serviced immediately, please submit a photo of the updated card to Charlie.Yang@dss.ca.gov.
