Facility Evaluation Report
Unannounced Annual visit made out to this facility on 06/06/2025 by Licensing Program Analyst (LPA) Charlie Yang who was unable to meet with any facility staff members or the facility Licensee/designated Administrator at this time.
A phone call was made out to the facility Licensee/designated Administrator, Lilliana Bello, and this LPA was able to discuss the current state of this facility at this time.
It was learned that all of the residents and staff have been removed since this facility is currently pending under the licensing process to obtain a new licensed due to a change of ownership.
It was learned that there were no occupants in this facility at this time.
This LPA was unable to conduct the physical plant walk through for this facility, facility resident file review, and facility staff file review at this time.
The facility Licensee/designated Administrator, Lilliana Bello, stated that the new facility was waiting for the final inspection from the fire department so that the Prelicensing can be scheduled in the near future.
