Facility Evaluation Report
On 05/05/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Odion Dolorfino and Yashmin Ubaldo and explained the purpose of the visit. The purpose of the visit was to conduct an annual visit.
LPA Pascua asked Staff Members Dolorfino and Ubaldo to contact the Facility Designated Administrator (FDA), Leiscle Dolorfino Lugtu to inform them that CCL was present.
It was learned that this facility current has no residents in care. This facility is also licensed to serve and retain Level 6 clients from Valley Mountain Regional Center.
A tour of the facility was conducted to ensure that facility safety was in compliance, such as the fire extinguishers, smoke detectors, and carbon monoxide were present. All were present and in good repair at this time. A tour of facility bedrooms were conducted to verify that there are currently no residents in care.
LPA advised Yashmin Ubaldo to inform FDA Dolorfino to notify the department once facility obtains residents.
Based on the observations made, there are no deficiencies observed during this annual visit.
An exit interview was conducted and a copy of this report was provided to the facility.
