Facility Evaluation Report

Unannounced case management visit made out to this facility on 04/03/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Melinda Womble, who was briefly interviewed at this time.

The purpose of this case management visit was to address other issues and incidents with the facility designated Administrator at this time.

The following issues were discussed with the facility designated Administrator at this time:

This facility did not report COVID case(s) which occurred on 06/05/24.

This facility did not report multiple falls that occurred between 06/05/24 and 06/11/24.

This facility did not report the hospitalization of a resident on 06/11/24 and the medication error that occurred on 05/18/23.

The following deficiencies were observed and cited on the following LIC 809-D pursuant to Title 22 Rules and Regulations, Health and Safety Codes.

Appeal Rights were printed and a copy was given to the facility designated Administrator at this time.
