Facility Evaluation Report

Unannounced case management visit made out to this facility on 06/24/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Melinda Womble, who was interviewed at this time.

The purpose of this visit was to tour the facility since this facility, and it's representative, have made the decision to close and relinquish this facility license at this time.

It was learned that there used to be (3) residents, (R1, R2, R3) at this facility but have since been relocated to other licensed facilities at this time.

It was learned that these (3) residents were relocated at the beginning of June 2025 (06/01/2025).

The facility designated Administrator did provide the name of the new facilities where the (3) residents have been relocated to at this time.

A brief tour of this facility was conducted.

A tour of the resident bedrooms and restrooms was conducted. A review of the common rooms and areas intended for resident use was conducted at this time.

This LPA asked for the physical license for this facility but was informed that it was currently not present in the facility at this time. The facility designated Administrator stated she will go ahead and put it in the mail and send it into CCL at her earliest convenience.

There were no deficiencies observed or cited during today's case management visit.
