Facility Evaluation Report
Unannounced case management visit made out to this day program on 10/29/2025 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated Administrator, Deanna Smith, who was briefly interviewed at this time.
Also present was co-Director Justin Huiras.
The purpose of this visit was to review the additional space that was next to this current day program address. This day program was seeking additional space and an increase in the capacity to be able to now accept and retain up to 185 participants at any given time. This was an increase from the former capacity of 95 participants.
A tour of the additional space was toured at this time.
A review of the additional restrooms were conducted at this time. Hot water temperatures were taken and measured to make sure that they were within the allowed range of 105-120 degrees at all times.
Fire extinguishers, positioned throughout this entire space, were reviewed and observed to have been annually inspected and in compliance at this time.
Furniture and furnishings were observed to be present and able to meet the needs of the participants at this time.
Additional office spaces were observed to be present and in compliance at this time.
Additional classrooms were observed and toured at this time.
Additional exits were observed to be sufficient and able to meet the needs of the participants at this time.
There were no deficiencies observed or cited during today's case management visit.
This additional space has been observed to be in compliance at this time.
