Facility Evaluation Report

An Office Meeting was conducted on this day in the Sacramento South Regional Office via Microsoft Teams. The purpose of this meeting was to follow up with the facilities control of property and application for Change of Ownership. Present in the meeting was Licensing Program Manager (LPM) Liza King, Licensee/Administrator Guadalupe Yepiz, and Valley Mountain Representatives, Alondra Villagomez, Brian Bennett and Robert Fernandez.

The Department learned through the Secretary oif State that the licensee has opened an Corp "LA FAMILIA RESIDENCE INC " and has requested a change of ownership (CHOW) application. Licensee stated that an application will be submitted next week and agrees to collaborate with VMRC and CCL for the transition.

The focus of other concerns at this time include:

· Control of Property – Licensee confirmed that she and her spouse are owners of the property and maintain control of property.

· Administrator Designee – An LIC308 will be received by the Department appointing a designee in her absense.

· Notification requirements to residents and responsible parties.

An exit interview was conducted . No citations issued.
