LPA Lewis arrived at the facility announced and was met by the licensee ROSSELL-JENSEN, SHERRI .
The licensee contacted LPA Lewis via email notifying they wanted to close the facility on 08/28/2025. The licensee still has control of property and there the last resident left in January 2025. The licensee no longer wishes to operate. LPA Lewis inspected the facility to ensure there was no residents.The facility will close as of 09/24/2025. LPA Lewis collected the facility license.
