The inspector’s account

LPA Noel Wolf Petersen arrived at the facility today and met with facility manager Gerardo Varela, to follow up on documents that were requested via email on 11/10/2025, via telephone 12/08/2025, via email 12/08/2025 and 12/09/2025 of which include the following:

· all Board Meeting minutes and any Resolutions from October 01, 2022 through October 31, 2024

At the time of this visit, documentation as listed above has yet to be received.

The facility is being cited today inspection authority based on the failure to provide the requested documents (see 809D attached). LPA met with Gerardo Varela, reviewed the report, provided a copy as well as the appeal rights.
