Facility Evaluation Report

Licensing Program Analyst, LPA Noel Wolf Petersen arrived unannounced on 1/21/26 to conduct a Plan of Correction visit, regarding an earlier citation with a due date of 12/15/25. LPA Met with Facility Manager Geraldo Varela and explained the purpose of the visit.

LPA asked for the documents(coroprate licensee Gods Love Outreach Ministries board meeting minutes and board resolutions for the period of October 1 2022 through October 31 2024), the facility was not able to provide them by the POC date or today.

A civil penalty in the amount of 100$ per day is assessed for the failure to correct, from the date of 1/07/26 to today 1/21/26, a total of 1500$. A fine will continue to accrue in the amount of 100$ per day until the POC is implemented and proof of correction is submitted to the department for review.

Appeal rights were given, a copy of the civil penalty was given to staff, a copy of the report was read and given to staff. an exit interview was held.
