Facility Evaluation Report
Licensing Program Analyst (LPA) Noel Wolf-Petersen and Licensing Program Manager (LPM) Liza King arrived at the facility to conduct a case management visit as it relates to the most recent Department of Health Care Services (DHCS) inspection, as well as an annual inspection which is documented on a seperate facility visit report on todays date.
On April 30, 2025, Community Care Licensing received an Annual Review report from DHCS for the Program Recertification for the facility known as Short-Term Crisis Rehabilitation Program at G.L.O.M A.R.F. 5(GLOM5) which was completed on April 9, 2025. That report detailed the facility as being found out of compliance with California Code of Regulations(CCR), Title 9, Article 3.5, Section 532:
Specifically 532.2(f):”There shall be a written discharge summary perepared by staff and client, which includes and outline of services provided, goals accomplished, reason and plan for discharge, and referral follow-up plans.” Based on a review of two (2) closed client records, one of the client files is missing a goals accomplished section from the discharge summary, and two of the client files are missing a completed evidence of reason and plan for discharge section from the discharge summary.
After such a deficiency being found, per California Health and Safety Code(HSC), Title 22, Article 6, Section 80064, GLOM5’s administrator Jessica Guiterrez is discovered to be out of compliance with Administrator Qualification requirements. An exit interview was conducted, citations are listed on the D page and appeal rights were provided. .
