The inspector’s account

Licensing Program Analyst (LPA) Kesha Lewis arrived unannounced to conduct a case management visit on incident reports received. LPA met with RACHELLE REYES and explained the purpose of the visit.

Interviews were conducted regarding R1. The incident report received for R2 was dated 12/08/2025 regarding a resident being sent out to the hospital LPA requested current documents for R2. R2 is expected to return to the facility 12/12/2025 and LPA requested the updated care plan and discharge paperwork due by 12/13/2025 EOD. The facility’s dictations first day at the facility was 12/05/2025.

This matter is still under investigation LPA will return at a later date to deliver the findings.
